Annual Performance Evaluation – Professional Faculty

Intent:  This Performance Evaluation is intended as an annual review of accomplishments and goals, and is intended to promote communication and focus efforts consistent with further the Department, College and University missions.  Faculty members are to submit review material to the Department Head by December 1. 

This review consists of six sections to be filled out by the faculty member, and one section to be filled out by the Department Head.

1)      Summary of previous years’ accomplishments
2)      Faculty Evaluation worksheet
3)   Statement of goals for the coming year
4)       Oregon Invests – Summary of activities
5)      Review of Position Description (attached)
6)      Evaluation and Comments (to be completed by the department head)
7)      Faculty Member Response (optional)
